1987 CH-46A 2061 US 151913 : I think this may be the flight on a sight seeing trip in the PI flying down the Snake river with passenger's on board. The Helo rounded a bend and flew into a cable slam dunking the aircraft killing all on board. The Crew Chief was one of my instrucrers and good friend, Doug Groll. To Doug and all on board Rip. 1983 CH-46A 2127 US 152508 : II was a crew member on this flight and it was US Navy not Marine Cops. There were 4 crew members and I think seven passengers of which there was one fatality. We were making a single engine approach to the flight deck of the USS Santa Barbara (AE-28) when the second engine failed just before landing.

1975 CH-46D 2176 US 152554 : I was the plane captain of this aircraft while it was with HC-6. This plane was the most reliable in the squadron. It went several years past its due date for Paint and Repair (PAR) while serving on the USS Detroit (VERTREP) in the Med. We also took this plane on a detachment to Gitmo.

1985 CH-46D 2187 US 152565 : IWO JIMA (LPH-2) mishap confirmed. Pilot-in-Command (PIC) had been hot dogging all day. On last lift of retrograde PIC insisted that all VERTREP nets and pendants be strung together resulting in high hover with few visual cues to hold a stable hover. Acft drifted into yard arm and killed sailor on signal bridge. As previously stated fell onto AH-1T of HMM-261 and into forward gun tub.
1986 CH-46D 2197 US 152575 : Confirmed: Pallets of milk were weighed in kilos but crew thought it was lbs. Aircraft was over weight when attempting to land aboard the America. I personally talked to the crew following the accident as I was in their sister squadron, HC-6 and deployed to the “Med” at the same time.

1980 CH-46D 2285 US 153387 : I was aboard the USS Constellation (CV-64) and getting out of the Navy on 04aug80. Flew onto the Wichita via H-46 early that day for later transfer to Diego Garcia. Pretty sure I flew in on the same helicopter that went down; as the other H-46 was down for maintenance. Participated in the search and rescue efforts and remembered one survivor that had been in water close to 24 hours

1973 ch-53a 65-136 US 154873 : This a/c crashed a few days after Marine HMH-463 YH-11 went down in North Vietnam waters sweeping mines. Both crashes were a result of tail rotor pitch link failures. This crash resulted in all 53's being grounded and new more robust pitch links being installed. Neither crash caused fatalities

1969 hss-1 58-821 US 145666 : Irving Martin Goldrat was a member of a 3 man crew, together with the pilot and co-pilot. They were stationed at Naval Air Station Keflavik, Iceland. They were on a search and rescue mission to recover the crew and contents of a C-47 downed plane (mail, I think). The helicopter crashed in a field on a farm. The Navy claimed that the crash was caused by winds in the mountains. A friend told me that he had sent him living letters (cassette tapes) talking about the faulty transmissions they were dealing with in the helicopter(s).

1974 hss-2 61-012 US 148034 : I was with RVAH-13, Reconnaissance Attack Squadron Thirteen (Heavy) "Bats", flying RA-5C Vigilante ... and as I remember it, I was coming onto the flight deck to install a component into the jet, and I looked up and behind me as the HC was coming to land, and then noticed the SAM heading for the destroyer and I said 'oh shit' several times. The HC seemed to go full throttle and lifted and the SAM caught onto the heat signature from the HC and went directly into her and she exploded. I dove into the catwalk and saw the HC nearing the water below and it exploded again. I then picked up the tossed equipment and continued to the vigilante to install it. I was numb and in shock from the incident ... and am still dealing with it to this day ...

Day of crash it was transporting RCA contractors and mail between Roosevelt Roads and the Saint Croix Underwater Test Range. It went down a couple miles west of Frederiksted, St, Croix in over 3,000 feet of water. Crash site was on a portion of the Under Water Test Range. The next year portions of the aircraft were recovered by the DSRV Alvin.

1985 sh-3a 61-318 US 152124 : AK614. I was the first crewman (AW2 Brett Norton) and landed on the Coral Sea to do a refuel and hot crew swap. I had a terrible headache and could not figure out why. The crewmen that relieved us were Maes and Spigner. I told them that something was wrong with the helicopter. I exited the flight deck and heard “Man Overboard, aircraft in the water” over the 1MC. I looked over the side of the ship and saw 614 sinking. They assumed it was tail rotor failure. Maes was trapped in the tail, saw the flares ignite and swam towards them to escape. Spigner had the console come down on his legs and was able to pull it off and get out of the crew window. One pilot somehow went out through the overhead canopy. All crew survived and were rescued by another HS-17 Helo.

1969 UH-2A 69 US 149767 : 10aug69, UH-2C type with HC-1 Det31 on USS Bon Homme Richard (CVA-31). I was on my 3rd flight of the day in this helo and we had just trapped the last jet for this session and our pilot decided to do an auto-rotation from 3,000 feet. When we pulled out there was a series of violent jerks and the next thing we knew we were going into the water upside down. When we hit water we were on our side for several seconds, deployed the airbags, but the helo sank fast. All four crew were safe, a destroyer picked us up and we were returned to the carrier by another UH-2C. Our flight crew consisted of Lt Bill Bentley, co-pilot LTJG John Lovell, myself (AN Dave Wiczek) and ADJ-2 Jerry Vanlandingham who sustained injuries of a broken collar bone and a broken arm. a/c sank to the bottom of the Tonkin Gulf and to my knowledge was never seen again
