US Army Aviation 54-3007 : 007 was assigned to Co.A 504th Aviation Bn. (Germany) when I was attached in 1966. Co. A 504th became the 349th Aviation Company, 16th Aviation Bn. soon after. 007 was reassigned to another outfit. We missed it because of the tail number being "007". I remember it well. When it crashed it killed all on board. It threw a main rotor blade. All H-34's in NATO were grounded. All rotor blades were examined. I was the Crewchief on CH-34 54-3005 at that time. "005" had 3 out of 4 bad blades. It really woke all of us up.

US Navy 149692 : the helicopter had a sonar device attached to a 'boom' lowered in the water. The water tide or wave pulled the helicopter in. Crew of 4, the pilot and navigator were rescued; 2 radio operators lost. Helicopter actually went down ("NV64 went in the water") 11-May-1967. The two lost were declared dead 14-May-1967.

US Marine Corps 148819 : 06dec67 I was crew chief on this plane ( had the evil eyes painted on the nose ) it was hit with automatic weapons and one RPG anti rocket, the aircraft was destroyed due to the area being inaccessible for recovery operations. I crewed this plane for about a year. We had a South Republic flag ( bars and stars ) hung in the cabin. I received 26 air medals, naval commendation medal, and a purple heart while I crew chief this plane. I have many memories both good and bad with her

US Marine Corps 153746 : I was co-pilot on this helicopter s last mission escorting a CH-53 on a resupply flight north of Phu Bai. We were hit by small arms fire and lost all engine oil pressure. Major Kyle Townsend, Commanding Officer of VMO-3 was the pilot and elected to put the aircraft on the ground before the engine seized. Although landing over a mile from where we were hit,we immediately came under small arms and later mortar fire from the VC. After the crew was evacuated, a Sparrowhawk team secured the area and a CH-53 attached the Huey to a sling for transport back to Phu Bai. Approaching the base perimeter and several hundred feet in the air, the Huey began to rotate and the Sea Stallion pilot pickeled his load before he lost control. The Huey was destroyed and the hulk of the fuselage remained where it fell. This was my second flight in country.
