Senior Taxi Aereo from May07 w/o 19aug11 over campos basin while serving Petrobas on route from P-65 offshore oil platform to Macae Airport (SBME) After takeoff, one of the tail rotor blades separated from the aircraft, hampering control of the aircraft by the crew. Seven seconds later, the aircraft could no longer be controlled, and crashed into the sea. 4 fatalities
