1963 107-ii 3 US N6673D : NYA d/d01jun62; w/o 14oct63 New York Airways Flight 600 crashed shortly after takeoff from Idlewild Airport (now JFK) en route to Newark via Wall Street due to lubricant oil contamination leading to component fatigue; 3 crew and 3 passenger fatalities
1965 107-ii 201 JP JA9501 : BV-107/II-2, c/n 201, disassembled and supplied as sub-assemblies to Kawasaki as KV-107/II-2, c/n 4010, 1962; delivered to Kawasaki Heavy Industries for certification trials as JA9501, 01Apr64; w/o 04Sep65 with 287 total flight hours.
1965 ch-113a 4011 CA 10417 : BV-107II-28, c/n 4011, ff?; del Cdn Army as CH-113A Voyageur 10417, May65; Struck off, after Category A crash near CJATC, Rivers, Manitoba, 27Sep65.
1966 ch-113a 4003 CA 10409 : asg 1 Transport Helicopter Platoon (1 THP), Royal Canadian Army Service Corps, Canadian Joint Air Training Centre, Rivers Manitoba. Crashed north of the town of Rivers
1967 107-ii 105 JP JA9503 : BV-107II-2 c/n 105; supplied in kit form to Kawasaki in 1962, built as KV-107II-2, c/n 4004, del KHI-Air Lift Inc as JA9503, 20May63; used in filming James Bond 007 movie "You Only Live Twice"; w/o 05Aug67 with 1872 total flight hours.
1968 107-ii 103 JP JA9502 : BV-107II-2 c/n 103, orig reg N4625G; supplied in kit form to Kawasaki in 1962, built as KV-107II-2, c/n 4002, to Kawasaki Helicopter Systems as JA9502; served with Tokyo Airlines, unk; served with Kanki Airlines, unk; w/o in Japan 16Apr68.
1968 hkp4b 502 SE 04062 : del to Swedish Marinen as Hkp4B #04062, 1964; w/o 06Sep68; crashed in the sea south of Huvudskär due to rear rotor mech failure. Four crew and 6 (of 8) passenger fatalities.
1974 ch-113a 4007 CA 11313 : 11313 crashed near NAMAO, ALBERTA during night maneuvers on March 19, 1974 (Not in BC as noted). The crew of three, Capt. Ronald Raymond Fleming (36), Captain Ronald George Davidson (31) and MCpl Malcom Ross Caton (29) all perished in the crash. An investigation revealed the mishap resulted from internal rotor blade spar erosion and cracking resulting in about three metres of one of the forward blades detaching in flight. This resulted in a catastrophic mid-air breakup of the aircraft.
1982 107-ii 6 US N6676D : sold to Columbia Helicopters as N6676D, Jan72; broke up in flight while maneuvering during logging operations near Cascade, ID on 14Oct82. Suffered rotor synchronization shaft failure and crashed to ground, 2 crew fatalities.
1992 ch-113a 4005 CA 11311 : xfer CAF, reserialled as 11311, 1968; conv SARCUP std unk; Serving with No. 442 Squadron at CFB Comox, BC, when crashed on 30Apr92. Was hoisting stranded hikers when engine failed at high altitude causing crash at 5,000 ft level in mountains near Bella Coola, Vancouver Island.  One fatality.
1996 hkp4a 407 US N196CH : BV-107II-14, c/n 407, ff ?, reg N4636G; del to Swedish Air Force as Hkp4A #04457; sold Oct 1991 to Columbia Helicopters as N196CH. Written off October 04, 1996 in Canby, OR due to faulty maintenance procedures on flight control system, 3 fatalities.
1998 ch-113 305 CA 11305 : xfer CAF, reserialled 11305, 1968; conv to SARCUP std, unk; suffered in-flight engine fire and crashed near Marsoui, Que, 02Oct98.
2001 107-ii 106 PG P2-CHA : lsd CHI-PNG as P2-CHA, unk; rtnd CHI as N186CH, unk.
2005 hkp4a 402 CA C-GHFT : lsd Helifor as C-GHFT, unk; broke up in flight while maneuvering during logging operations near Bella Bella, Canada, 03Nov05, 2 fatalities.
