1967 CH-46A 2191 US 152569 : The data is incorrect. The breakup was caused by a rotor blade failure due to battle damage. The aircraft shook itself to pieces. I still have a small piece of the aircraft, because I did the bullet hole repair to the rotor blade. The blade failed after apron 100 flight hours. The date is correct.

1967 h-34a 58-346 US 54-3007 : 007 was assigned to Co.A 504th Aviation Bn. (Germany) when I was attached in 1966. Co. A 504th became the 349th Aviation Company, 16th Aviation Bn. soon after. 007 was reassigned to another outfit. We missed it because of the tail number being "007". I remember it well. When it crashed it killed all on board. It threw a main rotor blade. All H-34's in NATO were grounded. All rotor blades were examined. I was the Crewchief on CH-34 54-3005 at that time. "005" had 3 out of 4 bad blades. It really woke all of us up.

1967 hss-2 61-105 US 149692 : the helicopter had a sonar device attached to a 'boom' lowered in the water. The water tide or wave pulled the helicopter in. Crew of 4, the pilot and navigator were rescued; 2 radio operators lost. Helicopter actually went down ("NV64 went in the water") 11-May-1967. The two lost were declared dead 14-May-1967.

Recovered Lt. Michael Burns but lost to sea Lt. Bostic and 3rd class David Dutko. Lt Burns died later of massive trauma.
