1996-03-19 | A KNCA | accident w/o MCAS New River “ Caught fire just after takeoff. It was determined that the APU Start Module cracked and sprayed atomized hydraulic fluid into the intake of the #2 Generator. The aircraft landed on the over run of Runway 08 and almost burned to the ground before CFR put the fire out...all that was left was the floor of the aircraft, the cockpit and the Vert Shaft. All crew and passengers safely exited the aircraft ” | |
B KNYL | accident witness “ Your information is wrong. 154821 was lost at MCAS Yuma AZ during WTI. A MAWTS-1 crew was flying during the morning hours for TAC support and were transporting approx. 10 PAX. On takeoff a hydraulic line going to the APU start module developed a pin hole and cut into the #2 Generator resulting in fire. The MAWTS-1 CC evacuated the PAX and waited on CFR to arrive. Due to the length of travel for CFR 154821 was nearly destroyed (Aft XMSN/ Canted Bulk head/ Engine Bay/ 410 forward upper cabin). HMM-261 was assigned to conduct the mishap investigation and responsibility of movement from mishap site to the hangar for the investigation. We HMM-261 used the pettybone crane and removed the aft rotor head and vertshaft (palmtree) and then reinforced the stubwings by applying chains and cargo straps so we could tow it to the hangar. 154821 was stripped of all usable parts and was removed from the inventory and towed to the DRMO lot to await disposition following the investigation. ” | ||
