11313 crashed near NAMAO, ALBERTA during night maneuvers on March 19, 1974 (Not in BC as noted). The crew of three, Capt. Ronald Raymond Fleming (36), Captain Ronald George Davidson (31) and MCpl Malcom Ross Caton (29) all perished in the crash. An investigation revealed the mishap resulted from internal rotor blade spar erosion and cracking resulting in about three metres of one of the forward blades detaching in flight. This resulted in a catastrophic mid-air breakup of the aircraft.
