1970 ch-54a 64-047 US 68-18445 : w/o , destroyed at Sanford AAF, Vietnam. Aircraft was undergoing M.O.C. after maintenance. While being run up, aircraft picked up and nosed over due to wrench left in flight control system. Aircraft was destroyed when M/R blades contacted the runway.

1998 ch-54a 64-051 US N64KL : sold to Heavy Lift helicopters, Ketchikan, AK as N54KL, 1993; rereg N64KL, 1995; w/o in Yucca Valley, CA 18Jul98 during ferry flight. one rotor blade failed and separated in flight, causing explosion and crash, 3 fatalities.
2001 ch-54a 64-030 US N18428 : sold Heavy Lift Helicopters as N18428, unk; w/o when crashed into hillside while fire-fighting in Sardinia, Italy 18Jul01.
2003 ch-54b 64-086 US N223AC : sold Erickson Air Crane as N223AC unk; conv S-64F unk; w/o 100nm E of Bintutu, Sarawak, Malaysia, 01Jun03 when helicopter was destroyed following a forced landing after a loss of control during logging operations. A post-crash fire consumed the wreckage. The pilot suffered serious injury and the co-pilot minor injury.
