Feb 5, 2009	Deployment	off Mokauea Island ran aground on a reef off Oahu. Some equipment lightered off and ship eventually floated off with tug assistance on 9 Feb for repairs. Accident
Mar 2009	Refit	Extensive repairs to underside of ship, including replacement of bow-mounted sonar dome, the shaft, running gear and propellors were refurbished, a repaint for the underwater hull and structural repairs to her tanks and cracking in the superstructure.
Jun 24, 2011	Deployment	NS Pearl Harbor departed Mike Pier at Pearl Harbor for 7 month Western Pacific and Middle East deployment. 1st deployment since running aground in Feb 2009.
