US Navy 152575 : crash was during VOD (Vertical Onboard Delivery) not VERTREP (Vertical Replenishment). The helo was delivering internal cargo (Pallets of Milk) when the stubwing struck the deck edge causing the helo to roll on its starboard side
US Navy 152575 : Confirmed: Pallets of milk were weighed in kilos but crew thought it was lbs. Aircraft was over weight when attempting to land aboard the America. I personally talked to the crew following the accident as I was in their sister squadron, HC-6 and deployed to the “Med” at the same time.

US Marine Corps 155317 : HMM-268, 11 MAG; It was on a routine night operation when it plunged into the sea about 10 miles southwest of Cubi Point Naval Air Station, Philippine Islands. Killed in the crash were 1st Lt. Michael Milnes of Ann Arbor, Mich., the co-pilot, and crew chief Cpl. Kenneth J. Badeaux Jr. of Baton Rouge, La. The pilot, Capt. Paul S. Desantis of Norwood, Mass., was picked up by rescuers and was hospitalized in stable condition. A 17-hour search of the area failed to locate either Milnes or Badeaux

US Marine Corps 157689 : 22 onboard. 9 died including Corporal Rory Fowler (Crew Chief) and Captain Dale Elzey (Pilot). The crash was considered a result of a SAS systems failure. The aircraft landed starboard nose down, then fell off of the deck of the USS Saipan into the North Sea off the coast of Bodo, Norway. The back half of the aircraft was recovered on the scene, although the nose of the aircraft was believed to have sank to the bottom. The Co-Pilot Lt. Mauzey and a new Crew Chief survived, both with injuries

US Coast Guard 1473 : I was the radioman on duty at the air station when this crash occured. They were flying out to bring in a child with a head injury from a village. We sent a C-130 to look for them after they failed to make radio contact. The c-130 found the wreakage burning. I remember how the CO of the airsta looked right through me as I informed him there were no survivors, like his mind was a million miles away, with his lost crew.

Cracks were discovered in the mounts of other aircraft following this incident. The passenger door departed the aircraft due to the violent vibrations created by an unsupported/unbalanced driveshaft.
